Internship Proposal Form

CPH 709 Global Health Internship

Internship placements will require completion of a proposal to be submitted to and reviewed by the Global Health certificate director.  The internship proposal shall include a proposed work plan, with activity and learning objectives, and a mentorship plan which should include a UK-GH associate faculty supervisor and a local, qualified mentor or supervisor in the host location.  Approval of the internship proposal will also include consideration of the student’s background, interests and language capabilities. 

In addition to the field experience, before travelling the student will complete pre-departure orientation and preparation requirements of the Office of International Affairs for students studying and/or working abroad.  Upon returning to the U.S., the student must report on the experience in a form of a written report approved by the UK-GH supervisor, for the completion of the internship prior to the assignment of a grade.

  • I understand that I must turn in a fieldwork report upon completion of my fieldwork
  • I understand that my local mentor/preceptor must submit an evaluation form upon completion of my internship
  • I have assessed whether my project needs IRB approval and have taken the appropriate steps to gain IRB approval if needed (any required IRB approval needs to be obtained before internship begins)
  • I have taken the required Global Public Health and Epidemiology (or equivalent) courses
  • My project and my participation have the approval of my local mentor/preceptor
  • I understand that before traveling to my internship site I have to complete the pre-departure orientation and preparation requirements through the Office of International Affairs
  • I understand that for internship credit, upon returning to the University of Kentucky, I must report on my project and experience in the form of a written report approved by the UK-GH supervisor for the completion of the internship prior to the assignment of a grade.
