Students applying to the Doctor of Philosophy in Epidemiology and Biostatistics must apply through the Centralized Schools of Public Health Application Service (SOPHAS), as well the University of Kentucky Graduate School.

The Admissions Committee requires a minimum undergraduate GPA of 2.75, and a minimum graduate GPA of 3.0. Official GRE, GMAT or MCAT scores must show verbal and quantitative scores at or above the 50th percentile. A previous master's degree is preferred.


Applications are due to SOPHAS by February 1

Applications requirements:

  • Completion of Calculus I and Calculus II; 
  • Personal statement;
  • Official GRE, GMAT or MCAT scores - Test requirements are waived for applicants who have earned from an accredited US institution any of the following degrees: PhD, MD, DMD, DDS, PharmD, DVM, DO, DNP or DSc; 
  • Official TOEFL scores (for international students from non-native English-speaking countries); 
  • Transcript(s) for all prior graduate or undergraduate education - International (non-US) transcripts must be evaluated by WES
  • Contact information for 3 recommenders, and
  • A resume or CV.


University of Kentucky Graduate School

You will also need to apply to the University of Kentucky Graduate School.

The UK Graduate School deadline is February 1 for international students, or at least one month before starting classes for domestic (US) students.

All information received from your SOPHAS application is sent to the Graduate School by the College of Public Health. When completing your application for the Graduate School, you can upload the documents you used for your SOPHAS application, or you can submit blank sheets of paper.
