Authorized UK departments submit this form to the University Registrar. This form must be completed by a department when they want to: (a) change a course in the printed schedule, (b) create new sections for courses in the printed schedule, (c) drop a course or (d) change the credits, days, hour, building, or room number for a course.
Submit a completed, and signed, pink sheet to the Registrar’s office.
Faculty or department contacts the AAA with a course they need to add, change (ie: time, day), or cancel. The AAA completes the pink sheet & sends it to the department chair for approval signature. Once the AAA receives the form back with department approval, the Associate Academic Dean signs to approve. Once the form is completed, it is sent to the Registrar’s office (course.scheduling@uky.edu) for the adjustment in the system to be made.

The University of Kentucky College of Public Health is undergoing a review by the Council on Education for Public Health (CEPH) for re-accreditation. Part of this process includes the invitation to provide third-party comments. Anyone, including students, alumni, employers, community partners, etc., is allowed to share relevant information about the college directly with CEPH.
