Description

The Degree Program Change (Significant) review procedure is to ensure a standard and efficient review of any program change proposals within the College of Public Health.

Definition

Degree Program Change (Significant)

The most common types of significant changes are listed below. If the proposal does not include one of these components, instead submit a "change" program, not a "significant change" proposal.

  • New track, concentration, or specialization

  • Online delivery for an entire degree

  • Online delivery for a track, concentration, or specialization

  • Changes to admissions requirements

  • Changes to progression requirements

  • Changes to graduation requirements

Suspension and closure of a program, a part of a program, or a modality are all significant changes, but those are submitted on a different form.

CPH Procedure

Proposers, please use the senate website, https://universitysenate.uky.edu/how-create-significant-change-proposal for guidance on how to create a proposal for a Degree Program Change (Significant).

  1. The College of Public Health department or faculty requesting the program change should contact the Associate Dean of Academic & Student Affairs (AD ASA) and/or the Academic Affairs Administrator (AAA) as soon as their program change is initiated.

  1. The AD ASA (or his/her delegate) submits the Substantive Change Survey from the Office of Strategic Planning & Institutional Effectiveness (OSPIE) to inform of a substantive change and to begin the process with SACSCOC. Online survey form available in the reference links below.

  2. The AD ASA and his/her delegate reviews the accreditation standards for the Council on Education for Public Health (CEPH), or other accrediting body, for compliance and/or concerns.

    1. The AD ASA and his/her delegate works with the CPH department or faculty:

      1. To meet the accreditation requirements for their proposal.

      2. To review the assessment plan for their proposal.

      3. To assess the evaluation of their proposal.

  3. The AD ASA and the CPH department or faculty work with the Business Office to ensure proper resources are available for the significant program change.

  4. The Originator of any program change will submit the requested change to their respective department or concentration level faculty for review and approval through a vote.

  5. The Originator of any program change will submit the requested change to their respective Program Faculty for review and approval through a vote.

  6. The Originator will contact the Academic Affairs Administrator (AAA) about the program change and relay the approvals from department and program faculty.

  7. The Originator and AAA work on required forms together

    1. Curriculog form.

    2. Curriculum workbook.

    3. Required letters.

    4. Other files.

    5. Identifies affected courses and updates syllabi with university required language.

    6. Questions if distance learning is an option, and ensures all documentation is executed.

  8. The AD ASA (or his/her delegate) submits the significant program change and any relevant courses for review to the academic program directors.

  9. The AD ASA (or his/her delegate) submits the significant program change, any relevant courses and any comments or concerns from the program directors for review to the appropriate committee based on current College Bylaws.

  10. The College Committee reviews and approves or denies with an official vote.

    1. If approved, it is sent to Faculty Council.

    2. If denied, it will be returned to the Originator with comments.

  11. The Faculty Council reviews and approves or denies with an official vote.

    1. If approved, the AD ASA designee approves the significant program change proposal in Curriculog at the college level and the proposal is routed to the appropriate Council to begin the University level review of the program. (NOTE: once outside the College, this process can take 6-9 months before approval) 

    2. If denied, the Faculty Council remits major course change to originator for revision(s) and resubmission.
