The following are all the forms for CPH employee to request different services:

  • Event Form: Allows CPH employees to request marketing and communications (MarCom) support for an upcoming event.

  • Hybrid Meeting Kit Form: Allows CPH employees access to the college's hybrid meeting kit.

  • News Form: Allows anyone to request MarCom coverage of a story, achievement, or announcement for news or promotional purposes.

  • Notification of External Speaker (Swag and Parking Requests Included) for CPH Faculty/Staff Only: CPH faculty and staff should use this form to notify the External Affairs team of external guest speakers and request logistical support such as speaker gifts and parking arrangements.

  • Photography Request Form: Allows CPH employees to request a photographer to cover an event.

  • Photo Submission Form: Allows anyone to submit photos to MarCom for archiving or use in social media and other communications.

  • Request Form for an External Alumni Speaker (Internal Use Only): Allows CPH employees to request a speaker from the Speakers Bureau list for a class, event, workshop, or mentorship opportunity.

  • Speaking/Engagement Interest Form: This form is for alumni and external professionals to express interest and availability in contributing their expertise through guest speaking, panels, workshops student mentorship, or collaborative college initiatives.

  • Swag Request Form: Allows CPH employees to request promotional items (swag) for events, outreach, or recruitment activities.

  • Website Profile Form: Allows CPH employees to request the creation of a new profile, update an existing profile, or removal a current profile from the website.

  • Course Technical Needs: Allows CPH employees to request Canvas and/or Echo Technical Assistance.
