- ADA Accommodation Form
- Employee reasonable accommodation form.
- Elder Care Leave
- Employee Education Program
- This fillable form allows UK Employees to enroll in the Employee Education Program (EEP) at UK.
- Employment Verification
- Family Education Program (FEP) Enrollment Form
- As a UK employee, you have the opportunity to enroll a family member in the Family Education Program through UK.
- Family Medical Leave Application
- Submit forms to Lindsey Powell by emailing lindseyhpowell@uky.edu or faxing to 859-257-2010.
- Hybrid/Remote Work Request Form
- Once you've discussed your schedule with your supervisor, you'll need to complete our remote and hybrid work agreement form.
- Local City Tax Form
- For employees who do NOT work in Lexington, KY.
- Local City Tax Form - Continuation
- Change work location for employees working outside of Fayette County or for employees working in other states.
- Local City Tax Form - Royster
- This form is used to exempt RA/TA/GA's from the local city tax under the Wimberly C. Royster Memo.
- Parental Leave Form
- If you do not qualify for Family Medical Leave, use this form to request two weeks of paid parental leave.
- Phased Retirement Agreement Faculty & Staff
- This form allows staff or faculty of UK to enter a Phased Retirement process.
- Seasonal/Temporary Reduced Hours
- Send a proposal for these types of flexible work: adjusted hours, compressed work week or reduced hours.

- This form allows UK Staff to apply for use of the Staff Shared Leave Pool allocation.
